**$1,000 Sign-on Bonus **
We are seeking a compassionate and skilled Social Worker to join our team! The Social Worker provides comprehensive support through assessment, planning, implementation, and evaluation of patient and family needs. This role also involves participating in bereavement services when necessary.
Schedule: 30 hours per week, 1st shift weekdays! Days each week can be flexible, with every other Monday as a requirement. Weekend work - one shift per month or one weekend every other weekend (flexibility on what option works best for you).
Key Job Duties:
- Develop and continuously assess patient care plans and psychosocial needs.
- Provide supportive, educational, and crisis counseling to patients and families.
- Maintain ongoing communication with patients, families, facility staff, and team members.
- Coordinate and participate in care planning meetings.
- Collaborate with the interdisciplinary team to document and update care plans.
- Use flexibility and creative problem-solving for complex psychosocial needs.
- Advocate for patients and support informed decision-making.
- Educate patients, families, facility staff, and professionals about end-of-life psychosocial needs.
- Assess bereavement risk and plan for bereavement care for family members.
- Foster positive relations within the organization and community.
- Adhere to the Social Work Code of Ethics and ethical principles.
- Participate in interdisciplinary team meetings.
- Maintain relationships with contracted facilities.
- Engage in community education and networking.

Qualifications:
- Bachelors degree or Master of Social Work (MSW) from a school of social work accredited by the Council of Social Work Education OR,
- Bachelor's degree in psychology, sociology, or other related fields related to social work, and is supervised by an MSW along with 1 year of social work experience in a healthcare setting.
- College or university must be accredited by the Council on Social Work Education.
- One year of social work experience in a healthcare setting.
- Certified or licensed as a Social Worker by the State of Wisconsin required. Applicants must have or obtain certification or licensure as a Social Worker by the State of Wisconsin before an official job offer is extended. Unity will reimburse the exam fee for candidates who are newly obtaining their license.
- Valid Wisconsin Driver’s License, reliable transportation, and proof of current automobile insurance coverage.
- Internet access to accommodate charting and laptop synchronizing capabilities.
